Invisalign Comprehensive

Patient Chief Concern
My teeth are crooked, and front teeth don’t show enough when I smile.

What learning would you share with other Invisalign providers?
Invisalign aligners can effectively achieve arch expansion, and when combined with well-planned IPR, it can significantly improve dental crowding. In addition, with proper attachment placement, extrusion movements can be predictably achieved without the need for elastics.

What contributed most to this treatment being successful?
Successful arch expansion, anterior proclination and interproximal reduction were performed in both arches to create space for relieving crowding. Additionally, upper anterior teeth were successfully extruded to improve incisal display during smiling.

Clinical Indications

  • Anterior crossbite
  • Class I
  • Crowding
  • Facial/dental midline descrepancy
  • Mandibular midline deviation
  • Narrow arches
  • Rotations

Modalities

  • Extrusion
  • Intrusion
  • IPR
  • Leveling Curve of Spee
  • Proclination
The initial overbite was acceptable, but the patient desired better upper incisal display. Therefore, upper incisor extrusion and lower incisor intrusion were planned to enhance incisal show while maintaining an ideal overbite. In addition, the patient presented with a significant mandibular deviation to the left, resulting in a left-deviated lower midline. By strategically planning the location of IPR, the lower midline was successfully corrected.

Summary

Corrected crowding and anterior crossbite by utilizing space gained through anterior proclination, interproximal reduction (IPR), and arch expansion. Corrected the lower dental midline deviation to the left by strategically allocating the amount of IPR. Achieved maximum interdigitation with a stable Class I canine and Angle Class I molar relationship and attained an ideal overjet and overbite.
