What learning would you share with other Invisalign providers?
To effectively close spaces, ghost IPR can be planned to achieve tight contacts. Invisalign aligners not only can close spaces between teeth efficiently but also allow for individual tooth torque control and intrusion simultaneously. To maintain proper torque during anterior intrusion, power ridges can be applied.
What contributed most to this treatment being successful?
The patient had previously had orthodontic treatment with extraction of the first premolars six years ago, in another clinic. However, the diastema relapsed two years after treatment. To effectively address the chief complaint of the diastema, ghost IPR was planned between #11 and #21. Since there was also spacing in the mandibular anterior region, ghost IPR was applied there as well to achieve tight contacts. To correct the Class II canine relationship on the left side, Class II elastics were used unilaterally on the left. Due to a tendency of anterior deep bite, bite ramps were placed on the four maxillary anterior teeth, and anterior intrusion was performed. Overcorrection of the intrusion of the anterior teeth and extruding the posterior teeth was designed, resulting in intentional edge-to-edge incisal relationship at the final aligner stage. At the same time, the posterior teeth were guided into strong occlusal contacts to prevent relapse of deep bite.
