What learning would you share with other Invisalign providers?
In adolescent patients with a severe Class II molar relationship, Invisalign aligners are effective and can simultaneously achieve mandibular advancement, dental alignment, and resolution of spacing, etc. Also, in patients with a severe deep bite, anterior intrusion can be set close to an edge-to-edge bite, and strong posterior occlusal contacts can be settled through overcorrection to prevent relapse of deep bite. Class II elastics can contribute to the mesialization of the mandibular first molars, making them an effective tool for space closure.
What contributed most to this treatment being successful?
The patient presented with a Class II molar relationship and a severe deep bite. To achieve a Class I molar relationship, treatment was planned using mandibular advancement (MA). Additionally, due to the presence of spacing in both the maxillary and mandibular anterior regions, space closure was also emphasized. To address the severe deep bite, significant intrusion of the maxillary and mandibular anterior teeth was performed. Simultaneously, anterior retraction was carried out to close the existing spaces. In the final stage of treatment, the overbite was planned to be reduced to 0 mm, and strong posterior occlusion was established. For the maxillary anterior spacing, a general retraction of the anterior teeth was planned. In the case of mandibular anterior spacing, the mandibular canines and first premolars were moved mesially, and retraction of the four lower incisors was performed. The remaining interdental space distal to the mandibular first premolars was gradually closed during the second phase of treatment using Class II elastics, which facilitated the mesial movement of the mandibular first molars.
